Working With Alliance

Alliance Nursing is driven by a single ideal—to provide quality, compassionate nursing that preserves the dignity of the individual in a home setting. We do everything in our power to ensure a positive experience for every client and family we serve.

We are licensed to provide hourly care for long term, chronically ill, technology- dependent or developmentally disabled children and adults in their own homes or in one of our Adult Family Homes, and maintain a dedicated nursing staff to provide this care.

Establishing the relationship

Step 1
It all starts with a phone call. Parents or spouses, case managers or discharge planners may call to refer a new client. It is generally best to call three or four weeks prior to discharge so that appropriate arrangements can be made.

The majority of case managers and discharge planners know Alliance Nursing from our reputation in the community, and are usually familiar with our specialty services. Families are encouraged to call for full details about our services and the process.

Step 2
In receiving a referral for a new client, we review the client’s needs to ensure a fit with our services. If the need is not for medically intensive home health care, we may refer to more appropriate providers.

Step 3
A review of provider home health benefits is completed to ensure funding for hourly nursing care. Alliance Nursing accepts a variety of funding sources to provide hourly care at home or in a group home.

Step 4
We consult physicians’ orders, and complete a thorough needs assessment and treatment care plan with the client, parents and/or spouse. This becomes the blueprint for directing care, and is signed by the physician and updated every two months. We then assign one or more nurses to the client. Our nurses frequently work with just one or two clients, and we keep their shifts as consistent as possible.

Step 5
Prior to the client’s discharge from the hospital, an orientation is coordinated for the new nursing staff to have an overview of nursing care needs, understanding medical history and nursing goals. As the transition from hospital to home occurs, the Alliance Nursing Clinical Supervisor stays in close contact with the family and nursing staff to coordinate a smooth transition.
